EverHaven Therapy provides in-home Applied Behavior Analysis (ABA) therapy services for children with autism spectrum disorder and other developmental disabilities. Our services include:

- Parent and caregiver training
- Progress monitoring and reporting
- Coordination with schools and other providers
Consent for Treatment
Before beginning ABA services, parents or legal guardians must provide informed consent for treatment. This includes:
- Understanding the nature and goals of ABA therapy
- Acknowledging the potential benefits and risks
- Agreeing to actively participate in the treatment process
- Consenting to data collection for treatment planning and progress monitoring
- Authorizing communication with other treatment providers as needed
You may withdraw consent at any time by providing written notice to our office.
Client Responsibilities
To ensure the best outcomes for your child, families agree to:
- Provide accurate and complete information about your child's history and needs
- Maintain a safe and suitable environment for therapy sessions
- Ensure a parent or authorized adult is present during all sessions
- Notify us of any changes in your child's health, medication, or circumstances
- Participate in parent training and implement strategies at home
- Communicate openly with the treatment team
- Provide at least 24 hours notice for appointment cancellations
Scheduling and Cancellations
Consistent attendance is essential for effective ABA therapy. Our cancellation policy is as follows:
- Please provide at least 24 hours notice for cancellations
- Repeated late cancellations or no-shows may result in schedule changes
- We understand emergencies occur and will work with families on a case-by-case basis
- Excessive cancellations may impact insurance authorization for services
Insurance and Payment
EverHaven Therapy works with many insurance providers. Regarding payment:
- We will verify insurance benefits before beginning services
- Families are responsible for any co-pays, deductibles, or non-covered services
- We will provide documentation needed for insurance claims
- Payment for any out-of-pocket costs is due at the time of service
- We offer payment plans for families with financial hardship
Confidentiality
We maintain strict confidentiality of all client information in accordance with HIPAA regulations and professional ethical standards. Information about your child will only be shared:
- With your written authorization
- For treatment, payment, or healthcare operations
- When required by law
- To prevent serious harm

Professional Standards
All EverHaven Therapy staff adhere to the highest professional and ethical standards, including:
- Board Certified Behavior Analysts (BCBAs) follow the BACB Professional and Ethical Compliance Code
- Registered Behavior Technicians (RBTs) are supervised according to BACB requirements
- All staff complete background checks and ongoing training
- Treatment is based on peer-reviewed, evidence-based practices
Limitation of Liability
While we strive to provide the highest quality services, EverHaven Therapy:
- Cannot guarantee specific outcomes from therapy
- Is not liable for progress that does not meet expectations
- Is not responsible for implementing recommendations outside of therapy sessions
- Requires families to maintain appropriate insurance coverage
Our liability is limited to the maximum extent permitted by applicable law.
Termination of Services
Services may be terminated under the following circumstances:
- Mutual agreement that treatment goals have been met
- Family request to discontinue services
- Non-compliance with treatment recommendations or these terms
- Safety concerns that cannot be adequately addressed
- Non-payment for services
- Loss of insurance authorization
When possible, we will provide reasonable notice and assist with transition to other providers.
